Chronic Diseases and Conditions

Priority Area Data Dashboard

Heart disease, stroke, type 2 diabetes, cancer and illnesses related to tobacco use are among the most common health problems affecting people of all ages, socioeconomic statuses and ethnicities. Risk factors —lack of physical activity, poor nutrition, tobacco use, excessive alcohol use, the environment and social and economic factors—cause much of the illness, suffering and early death related to chronic diseases and conditions.

Chronic Diseases and Conditions

Priority Area Goals and Performance

The Chronic Diseases and Conditions (CD) priority area contains seven overarching goals with specific objectives developed for each goal. The goals for the CD priority area are:

  1. Reduce new cases of cancer and cancer-related illness, disability, and death.

  2. Improve cardiovascular health by reducing new cases, disability and death from heart disease, stroke, and other related illnesses.

  3. Increase palliative care access and use.

  4. Reduce the burden of diabetes and improve quality of life for all who have or are at risk for diabetes.

  5. Improve respiratory health.

  6. Promote the attainment and maintenance of health through health nutrition, physical activity, and supportive lifestyle behaviors.

  7. Increase access to, and utilization of, oral health care to reduce the burden and disparities or oral diseases and its impact on overall health among all Floridians.

The Indicator Performance bar chart presents a detailed view of progress across this specific Priority Area, with individual bars divided into segments that show the proportion of indicators within the Priority Area.
